Please use the link below to schedule an appointment.
- Enrolling Parent / Guardian must submit ALL required documents at the appointment for the registration packet to be processed. Incomplete paperwork will cause a delay in the student's start date.
- Pick up a hand-written enrollment form from CMS.
- Bring the completed form and other required docs to complete the enrollment process.
